Employment risks sheet
What should I do to fill out the occupational risk assessment form?

The Prevention and Protection Service needs the employment risk sheet to assess any exposure to occupational risks and to activate the related health surveillance if necessary.

Who completes the form?

  • Academic staff (full professors, associate professors, researchers, contract researchers, and post-doctoral appointees);
  • Technical-administrative staff;
  • Teaching and research collaborators (research appointees, research fellows, and collaborators, if the activity period exceeds 12 months or involves specific risks).

How to

  1. Refer to the type of work carried out at the Department.
  2. Access the Online Services → Employment risks sheet → Sheet.
  3. Select Sheet.
  4. Fill out Step 1 – Personal Data → Press “save” at the bottom of the page to confirm what is listed in the section.
  5. Fill out Step 2 – Job Activity → Press “save” at the bottom of the page to confirm what is listed in the section.

    The University Prevention and Protection Service will give you a feedback on any need for Health Surveillance.
